The Workplace Violence Assessment Questionnaire (WVAQ) is a violence assessment tool used by CMC, Inc. consultants and selected CMC, Inc. clients. A Board Certified forensic psychologist (ABPP) and a Supervisory Special Agent of the FBI (Retired) designed this technique. The WVAQ is not a formal psychological test. Rather, it is a violence assessment tool and research instrument. The WVAQ was developed by CMC, Inc. in an effort to integrate the scientific data that has been published to date regarding violence, with the practical, day-to-day need of forensic specialists performing forensic psychological evaluations assessing risk of violence in the workplace (and elsewhere). Using the WVAQ the forensic professional is better able to investigate, systematize, and understand a potentially threatening Target Person and thus arrive at a more competent Risk of Violence estimate.
The human resource director, supervisor, or business manager can also use the WVAQ. Specifically, the WVAQ is designed to help these individuals gather and focus their knowledge regarding a particular threatening employee or other menacing person (hereinafter termed Target Person), and, then, provide that information to a qualified threat of violence consultant.
The WVAQ is designed for use in those instances where the forensic investigator has access to a significant amount of information regarding the Target Person. Such instances include those where the Target Person is an employee, ex-employee, and/or the spouse, or significant other, of an employee. In these instances the investigator or business manager has the ability to obtain information from personnel records, interviews with fellow employees, staff, as well as the Target Person, if the latter is currently an employee. The WVAQ is not designed for use in those circumstances where the investigator does not have access to information regarding the Target Person. Examples of such instances are receipt of threatening mail or telephone calls, stalking, and/or destruction or sabotage of business equipment and property by person or persons unknown.
CMC, Inc. is now making this technique available to the public. Note, however, that when used to assess risk of future violence in adults the WVAQ should be used with appropriate training, or in consultation with an experienced forensic professional, and as part of a multi-method, multi-source forensic psychological evaluation procedure.

This includes the WVAQ Instruction Manual and Coding Forms, the WVAQ Software Manual, the WVAQ software application for Pocket PC, and an Intelligence Data Gathering Guide. The WVAQ software application for Pocket PC is made up of four modules: Target Person Data, Risk Factor Analyses, Action Guides, and Violence Risk Assessment.
